Its good that your concerned about your friend.
Becoming anorexic is a different process for everyone. Usually it is sparked by drastic changes in someones life or a lowering in someones self concience. For me it was the stress of my grandparents deaths within a month of each other.
It usually begins as a sort of diet. so you'd think from now on i'll not eat chocolate, then after a while i wont eat fatty foods and then now i won't eat carbohydrates. After that we may control how many calories we eat at each meal by decreasing portion sizes. By doing this they may start skipping meals. It becomes a stort of ritual and if you give in or fail then you've lost.
So in answer to your question in a way it does happen over time but it is usually triggered by something drastic.
As anorexics we try to become thin deliberatly but we didn't wake up one morning and say oh i'll become anorexic today. The desire to be thin is not always the only reason, for me i wanted some control over my life.
Please carry on trying to help your friend. Talk to people and get him/her the right help, fast. I'm lucky i'm slowly recovering from anorexia but i've seen how bad anorexia can get.
